Summary of Accident:
A construction worker was killed after falling from the scaffolding while climbing down for a rest. During the incident, the victim was laying the bricks at the gable end at a height of 6 metres. While he was climbing down the scaffolding, he slipped and fell down to the floor. He sustained serious head injuries and died on the way to the hospital.

 
Contributing Factors :
The victim slipped while climbing down from the scaffolding.

 
Results of Observation :
Lack of safe work procedures and monitoring by the employer.
