Incident Involving Passenger Lift

Recently, there was an incident involving a passenger lift which was used for the hotel car park. The passenger lift was exclusively for the workers but during incident, it was occupied by visitors. The incident resulted in injuries to the victims- some of them had broken arm dislocation. Investigation found that the passenger lift was occupied by 9 people although the capacity is only for 410 kg or 6 people. The lift is using 1:1 roping system and the location of motor is in the machine room which is at the bottom most level next to lift pit. The main component of the traction machine was damaged due to the unexpected movement of the pedestal towards the lift pit wall. Investigation also observed that the hydraulic buffer was fully compressed after it was hit by the lift car.
Suggestions:
- Lift passengers shall follow the safe allowable lifting capacity for the lift. If the overload alarm sounded, just wait for next lift.
- Lift maintenance company shall ensure that all safety devices for examples overload limit switch, governor, safety gear and brake are maintained in good working condition and periodically tested.
- Lift traction machine shall be securely anchored to the floor or any suitable location on the building which is in accordance to manufacturer’s specification.
- Examination on the error log data extracted from the lift controller shall be conducted periodically by the lift competent person. It will help to monitor for any problem and enable any remedial work to be carried out if it involves any safety feature for the passenger lift.
