Summary of Accident:
A factory worker was killed when his head was caught in the mask arm of a crusher. During the incident, the victim was trying to remove the scrap that was stuck inside while the machine was still operational. He was rushed to the hospital but died several hours later.
Contributing Factors :
The victim was trying to remove the scrap that was stuck inside without switching off the machine first.

Lack of safe work procedures for the handling of the machine and lack of supervision.
