Summary of Accident:
A palm oil mill worker was killed when he was caught between the shells and the fibre conveyor. During the incident, the victim was assigned to load the shells onto the conveyor by using a scoop. He was standing on top of a pile of shells when the scoop became stuck in the conveyor. He tried to pull the scoop out but he slipped onto the moving conveyor. The victim was completely pulled into the conveyor. He was rushed to the hospital but died on the way.
Contributing Factors :
The victim slipped when he was trying to pull out the scoop which was stuck in the conveyor.
Results of Observation :
Lack of safe work procedures.
