Summary of Accident:
Three workers were killed in an explosion in a water tank. During the incident, the three victims were applying anti-rust paint on the inside of the newly built water tank. The substance used by the victims was a combination of a type of protective coat and protective coating hardener as well as thinner. Suddenly, an explosion occurred while the victims were inside the tank. This resulted in serious injuries to the three victims who died at the scene.
Contributing Factors :
The substance used to paint the tank was made from flammable materials and during the explosion, the three victims were inside the tank.
Results of Observation :
Lack of safe work procedures for painting inside the tank.
