Summary of Accident:
A production worker was killed when he was trying to put out a fire in a silo. During the incident, the victim had opened the door of the silo and tried to put out the fire that was raging inside. While putting out the fire, the filter holder on the topmost part of the silo fell on him, causing the fire to engulf him. He fell to the bottom of the silo, at a height of about 3.5 metres. The victim died from serious burns on his entire body.
Contributing Factors :
The victim tried to put out the fire at the silo alone, which exposed himself to the dangers present around him.
Results of Observation :
Lack of safe work procedures.
